JAGUAR GYM — MEDIA & PRIVACY RELEASE

1. PHOTOGRAPHY & VIDEO CONSENT
I grant Jaguar Gym and its authorized representatives permission to photograph and/or record video of me (or the minor named in this packet) during classes, training sessions, events, demonstrations, and gym activities. I authorize Jaguar Gym to use such images and recordings for promotional, marketing, and educational purposes including but not limited to: social media platforms, website, print materials, and advertisements, without compensation unless otherwise agreed in writing.

2. PERSONAL INFORMATION & DATA PRIVACY
I understand and agree that Jaguar Gym collects personal information including name, date of birth, contact details, and health/medical information provided on the Student Information Sheet. This information is collected for the purposes of membership management, emergency response and safety, class scheduling, and gym communications. Jaguar Gym will store this information securely and will not sell, rent, or share it with third parties except as required by law or in a medical emergency.

3. HEALTH INFORMATION DISCLOSURE
I consent to Jaguar Gym sharing relevant health and physical limitation information disclosed on the Student Information Sheet with coaches, instructors, and staff on a need-to-know basis solely for the purpose of ensuring safe training conditions and appropriate modifications during classes.

4. COMMUNICATIONS CONSENT
I agree to receive communications from Jaguar Gym including class announcements, schedule updates, promotions, event notifications, and membership information via the contact information provided. I may opt out of marketing communications at any time by contacting the gym in writing.

5. SOCIAL MEDIA
I understand that photographs and videos may be published on Jaguar Gym's social media accounts and website. I may request the removal of specific content by contacting Jaguar Gym directly in writing.

6. WITHDRAWAL OF CONSENT
I understand I may withdraw my consent for photo/video use at any time by submitting a written request to Jaguar Gym. Withdrawal does not apply retroactively to content already published.

I opt out of photo & video use for promotional purposes.
